PDL Changes

 

Select Health made two substantial changes to the Preferred Drug List (PDL) for 2009. First, Lipitor® (atorvastatin) is now non-preferred and requires prior authorization. We have statin drugs on our PDL that patients can use an alternative that cover the LDL lowering capacity of Lipitor®. However, members currently taking Lipitor® 80mg can continue with their current therapy.

We understand there may be medically necessary reasons for your patient to continue using Lipitor®. In select cases and provided there is appropriate medical justification, we will honor prior authorization requests to continue patients on their current drug regimen involving Lipitor®. As a reminder, non-preferred medications and their PDL alternatives include:

Preferred Drug List Alternatives:

  • Lipitor® (Atorvastatin) 80mg
  • Lovastatin
  • Pravastatin
  • Simcor® (Niacin/Simvastatin)
  • Simvastatin
  • Vytorin® (Ezetimibe/Simvastatin)

Non-Preferred Medications:

  • Advicor® (Niacin/Lovastatin)
  • Altoprev® (Lovastatin)
  • Caduet® (Amlodipine/Atorvastatin)
  • Crestor® (Rosuvastatin)
  • Lescol® (Fluvastatin)
  • Lescol® XL (Fluvastatin)
  • Lipitor® (Atorvastatin) 10mg, 20mg, 40mg

The second change is with albuterol HFA inhalers. As you know, the CFC inhalers are no longer available. That leaves three branded HFA inhalers on the market—Proventil® HFA, ProAir® HFA, and Ventolin® HFA. Ventolin® HFA is our preferred product. ProAir® HFA and Proventil® HFA are non-preferred. All three branded products are the same strength (90mcg/actuations) and have the same amounts of actuations per inhaler (200). Ventolin® HFA is the only branded product that has a dose counter to accurately track albuterol use.
