Ambulatory Surgery Billing
 

Select Health of South Carolina now requires that all ambulatory surgery centers bill their claims on a HCFA 1500 form. Additional guidelines are found in the South Carolina Department of Health and Human Services' Physician Provider Manual. The information begins on page 2-166 under General Surgery Guidelines.
