Select Health and Medicaid follow CPT guidelines on multiple Evaluation and Management services on the same date of service. When a member is seen more than once on the same date, the work involved for all encounters should be compiled, and the code submitted should be based on the total work effort for that day.
For example, when observation is initiated in the course of an encounter in another site of service, all Evaluation and Management services provided in conjunction with initiating the observation are considered part of the initial care when performed on the same date. The level of care reported should include all services provided in all sites of service as well as in the observation setting using the hospital observation services 99217-99220 or 99234-99235. Or, when a member is admitted to the hospital from the office, all evaluation and management services provided in conjunction with that admission are considered part of the initial hospital care when performed on the same date as the admission. In this case, you would bill with the inpatient hospital services codes 99221-99223.
