Your Doctor

 

When your child joins First Choice Kids, you must choose a doctor from the First Choice Kids provider directory to get needed healthcare. This doctor is the primary care provider (PCP).You may choose a different PCP for each child in First Choice Kids or you may choose the same PCP for all of the children in your family. If you do not choose a PCP within 30 days of joining First Choice Kids, a PCP will be selected for your child. Click here to find a provider or call Member Services at 866.299.9594.

Your child’s PCP cares about your family’s health and arranges all healthcare. Whenever you need medical care, call your PCP’s office first — at any time, day or night. Your PCP will know how to help. If your child needs to go to a specialist or to the hospital, the PCP can make all the arrangements for you.

If Your PCP Leaves the First Choice Kids Network

When we know that your PCP is leaving the First Choice Kids network, we will notify you by mail and assign your child to a new PCP in your area. You may choose a different PCP by calling Member Services at 866.299.9594 within 30 days.  Your child will receive a new ID card within 10 days of getting a new PCP.

Get to Know your PCP

Make an appointment with your PCP right away, before your child becomes sick and needs medical care. Call the office of each PCP you choose for your family and make an appointment for a medical checkup. This appointment should be made within 30 days of joining First Choice Kids.

Keep your Appointments

Your child’s appointment time is reserved just for him or her. If you must cancel your child’s appointment, give your doctor’s office at least 24 hours notice. That way, others with health problems can be seen.

Changing PCPs

You may change PCPs by calling Member Services. We will help you choose a PCP over the phone. When you call to change your child’s PCP between the 1st and 25th of any month, the change will not take effect until the 1st day of the following month.

If you call after the 25th of the month, the change will not take effect until the 1st day of the 2nd month.

Your child will get a new ID card when you change PCPs. Please destroy your child’s old card. Call Member Services at 866.299.9594 for details on changing your PCP.
