As
part of the health assessment conducted when a family joins First
Choice, special medical concerns are identified that may benefit
from the attention of a nurse case manager. Members with chronic
conditions have access to outreach and education programs that
address certain diseases and medical conditions including high-risk
pregnancy, asthma management and diabetes.
