Choice also has a case management program staffed by registered
nurses with a variety of medical experiences. This program is
for members of all ages with serious medical problems or ongoing
chronic health care conditions. Case management assists them in
obtaining the best possible care to meet their needs. In partnership
with the primary care physician and other medical providers, the
case manager provides education to members and family, and uses
available resources working towards the best health outcome for
