Let Us Know Program

The Let Us Know program is a partnership between the provider community and Select Health of South Carolina in the engagement and management of chronically ill members.

Providers have clinical resources available for use in their care management. There are also support teams and tools available to assist in the identification, outreach, and education of members.

There are two ways you can let us know about chronically ill members

  1. Contact  our Rapid Response and Outreach Team (RROT)
    The RROT addresses the urgent needs of our members and supports providers and their staff.  The RROT consists of registered nurses and care connectors who are trained to assist members in investigating and overcoming the barriers to achieving their health care goals.

  2. Refer a member to the Complex Care Management Program
    Complex care management is a voluntary program focused on prevention, education, lifestyle choices, and adherence to a treatment plan. The program supports your care plan for members with chronic diseases, such as asthma, diabetes, or coronary artery disease.

    High-risk members will be assigned to a care manager for one-on-one education and follow-up.
