Earn rewards for good health!

You can earn rewards by doing things that help you stay healthy.

To begin earning rewards, all you have to do is complete the health screenings offered through our member incentive mailing. Then have your provider sign or stamp the form and return it to us in the envelope provided.

It’s that easy!

Better care, better rewards

Once you return your voucher and we verify that the provider has billed for the test, you will receive a text or an email with a link to redeem your reward. You will be directed to the First Choice by Select Health of South Carolina Healthy Rewards Marketplace. This is an external site where you can complete your reward selection. Click the link to claim your card and choose a gift card from a list of participating retailers. You may select to receive a digital or physical gift card.

The First Choice by Select Health of South Carolina Healthy Rewards Program allows you to choose gift cards from several participating retailers*.

Your rewards may not be converted to cash, and rewards expire if you lose First Choice by Select Health of South Carolina eligibility or are disenrolled from the plan for more than 60 days. Benefit terms and eligibility are subject to change. Gift cards cannot be used for purchasing firearms, tobacco, and alcohol. The gift card link will expire six months from the date the reward was sent. Once claimed, the gift card does not expire.  

Healthy Rewards are subject to change. First Choice by Select Health of South Carolina will notify you before the change happens.
